Documents required for registration of birth and death

Everything you need to apply for new birth registration

If the age is 0 to 45 days

1. EPI (Ticker) Card.

2. Photocopy of National Identity Card of parents along with online birth registration (Bengali and English mandatory).

3. House holding number and holding tax receipt will be required.

4. Mobile number of applicant/guardian.

5. 01 copy of colored passport size photograph will be required along with the form.


If the age is 46 days to 5 years

1. EPI (Ticker) Card/Certificate of Health Worker to be given in pad with signature and seal.

2. Photocopy of National Identity Card with online birth registration of parents (Bangla and English mandatory).

3. Certificate of Headmaster of the school with seal and signature where applicable.

4. House holding number and holding tax receipt will be required.

5. Mobile number of applicant/guardian.

6. 01 copy of Rakshin passport size photograph will be required along with the form.


If the age is more than 5 years

1. Certificate of Educational Qualification (PSC/ JSC or SSC) In case of absence of Educational Qualification Certificate Signature and Seal of MBBS Doctor of Government Hospital and signature in Column No. 7 (seven) of Birth Registration Application Form And seal is mandatory.

2. Note that for those born after 01/01/2001, photocopy of National Identity Card of parents with online registration (Bangla and English is mandatory).

3. Note that photocopy of National Identity Card for those born on/before 01/01/2001.

4. Photocopy of National Identity Card of parents in case of birth on/before 01/01/2001. If parents are dead then death certificate should be provided.

5. It should be noted that those born after 01/01/2001, if their parents are dead, first take the online birth registration and then take the death certificate.

6. House holding number and holding tax receipt will be required.

7. Mobile number of applicant/guardian.

8. 01 copy of colored passport size photograph will be required along with the form.

9. The documents attached with the application must be signed and sealed by the Head Teacher / Ward Councilor of the Government School.

10. Documents attached with the application Original copies must be produced at the time of submission of application.
